Addition of Registered Affix to Registered Name of Dog
Use to add the CKCSC-registered affix of a Regular member in good standing to the end of the CKCSC registered name of the dog.
Performance Event Number (PEN)
Use to register a Cavalier in the CKCSC, USA, Performance Event Registry who is ineligible for registration with the CKCSC, USA, Registry.
A Performance Event Number (PEN number) is not the equivalent of a CKCSC registration number. It is intended to allow Cavaliers not eligible for registration with the CKCSC to participate in CKCSC approved events, such as Matches, Junion Showmanship, Obedience Trials, Agility Events or other performance events. Under no circumstances may a Cavalier with a PEN number be entered in Conformation competition at a CKCSC Championship show and is not to be used for breeding. If a Cavalier with a PEN number is used for breeding, the offspring will be ineligible for registration with the CKCSC.
Register a Litter
Use to register a litter with a single sire where the sire and dam are not in the same ownership.
Register a Litter - Express
Use to register a litter with a single sire where the sire and dam are in the exact same ownership.
Register a Multiple-Sire Litter
Use to register a litter with two or more sires.
Register an Affix
Use to register an affix (kennel name).
Register an Individual Dog From Registries Recognized by the CKCSC, USA
Use to register a dog born outside the USA or a dog born in the USA not bred by a CKCSC, USA, Member and registered with the American Kennel Club.
Revocation of Restriction
Use to revoke a restriction from breeding or showing imposed by a breeder or former owner. This form must be received by the registration office PRIOR to any breeding taking place. Only the person who imposed a restriction upon a dog may revoke that restriction and no fee is required.
Do not use this form to request a restriction imposed by the CKCSC, USA, Board of Directors. Contact the Registration Office for instructions. Board imposed restrictions must be revoked prior to any breeding taking place and there is a fee to revoke a Board imposed restriction.
Stud Service Certificate
Use to attest to the mating of the sire and the dam.
Supplemental Litter Registration Breeder Information
Use to provide breeder information when the owner of the dam at whelping was NOT the owner of the dam at the time of mating.
Supplemental Signatures Form
Use when the dog has or will have more than two (2) owners of record in the CKCSC.
The CKCSC, USA allows a maximum of FOUR (4) owners of any dog or bitch. All owners must sign all forms submitted to the CKCSC, USA Registration Office. Since such forms allow space for only TWO (2) signatures, any form regarding a dog or bitch owned by more than TWO (2) people must be accompanied by a completed and signed Supplemental Signature Form with the box for the appropriate accompanying form checked.
Transfer of Affix
Use to transfer the ownership of an affix registered CKCSC, USA, to a Regular Member in good standing. Please read the CKCSC, USA guidelines for registering an affix to ensure your eligibility. Submission of this application does not guarantee the transfer of a Registered Affix. All Registered Affixes must follow CKCSC Policies and Procedures. Rev 2011.
Transfer of Dog
Use to transfer a dog. Same form is used for a restricted or unrestricted transfers.
