This is an All-Breed Health Clinic. Heart clinic priority times will be given to Cavaliers; other breeds will be scheduled for cardiac exams as time permits.
IMPORTANT: NO OFA Exam forms will be provided at the clinic except the multi-part form for Advanced Cardiac.
Please pre-fill your OFA applications online and either submit online (Eye Exams) or pre-fill, print and bring with you (all other exams) (https://www.ofa.org/application-forms).

Eye Certification: $35 - Owners MUST submit an Online OFA exam form. Paper forms are not available for eye cert
Please arrive 20 minutes before your scheduled appointment to allow eye drops to be administered for the exam
